Preparing for your Appointment
On your first visit, we will begin a permanent record. Please have the
following information with you when you come to our office:
- Your symptoms list including the date when your symptoms started.
- Your medical history of all previous diseases if any.
- Your history of all surgires that you had.
- Your list of all your hospitalizations, including date,
hospital's name, and reason for hospitalization.
- Copies of relevant medical records and X-rays, CT scans or MRI scan films.
Please bring films of tests previously taken such as CT scans, MRI, Doppler, etc.
- Medication List you are currently taking, including name of medication, dosage,
frequency, start date, and physician name who prescribed this medication. Also,
over-the-counter drugs, and vitamins list.
- Allergies or negative reactions you have had to any medications,
anesthesia, or dye used for testing purposes.
- Your referring physician's name, address, telephone and fax number.
- Pharmacy name and telephone number.
- Insurance card. Authorization for the visit if required.
Important Note
If this is your first visit to our office, please plan to arrive at
least 15 minutes early in order to complete a registration form.
