Patient Rights to Privacy and Confidentiality

Patients have the right to be treated with courtesy and respect.

  • Patients will be treated with appreciation of their individual dignity and protection of their need for privacy, to include the confidentiality of the medical record.
  • Patients have a right to expect that all communications and records pertaining to their care be treated as confidential by Florida Neurovascular Institute, except in cases such as suspected abuse or public hazards which are required by law to be reported.
Insurance
HMO/PPO Members
  • HMO/Authorization from your primary care physician required.
  • Copay due at the reception area before your visit.
  • Any unpaid balances are due prior to your follow up office visit.
MEDICARE/MEDICARE HMO
  • Prior authorization for your visit is required if you have a Medicare HMO.
  • 20% of Medicare allowable if you do not have a secondary insurance or your secondary insurance is not a medigap policy.
  • Outstanding balances are due prior to your follow up visit.

Scheduled Visit Cancellation

Please notify our office 24 to 48 hours prior to your scheduled visit otherwise, a cancellation fee of $40.00 will be billed to you.

Release of Medical Records

Release of medical information will not be permitted over the telephone.
A written authorization for release of medical record is required to be signed by patient / legal representative.

Medications Refill

Prescriptions will only be refilled during your doctor's visit. Not by phone or pharmacy call.

Test or Lab Results

Due to the new patient privacy act, we are unable to give out the test or labs results over the telephone.
Patient information, test or labs results will only be discussed at the time of your visit.
